We are looking for a dedicated Registered Nurse (RN) with proven home health experience, especially with OASIS documentation, for a 13-week full-time contract. Bring your expert clinical skills and strong commitment to quality care to a dynamic, mission-driven environment.
Schedule:
- Full-time contract, 13 weeks with potential for extension
- Monday through Friday schedule
- On-call responsibilities possible
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes
- Conduct comprehensive patient assessments, completing OASIS documentation accurately and promptly
- Partner with physicians and interdisciplinary care teams to create and carry out individualized care plans
- Administer wound care, manage medications, provide patient education, and monitor chronic diseases
- Communicate any changes in patient conditions to the broader clinical team
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Required experience with OASIS assessments
- Proficiency with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to independently manage a structured daily schedule and complete all assigned visits
