Job Description

A rewarding nursing opportunity is available for a dedicated Registered Nurse (RN) with home health expertise and OASIS experience. This 13-week contract offers the chance to provide vital in-home care while working a structured, full-time schedule Monday through Friday, with on-call duties as needed. This role allows you to demonstrate your clinical skills, precise documentation, and patient-focused care in a collaborative setting.

Key Responsibilities:

  • Deliver skilled nursing care and interventions in patients homes
  • Perform thorough patient assessments and complete OASIS documentation accurately and on schedule
  • Collaborate closely with physicians and interdisciplinary care teams to create and implement individualized care plans
  • Manage wound care, medication administration, patient education, and chronic disease monitoring
  • Communicate any changes in patient condition promptly to the clinical team

Qualifications:

  • Current RN license with at least one year of recent experience in home health nursing
  • Demonstrated experience with OASIS data collection is required
  • Familiarity with HomeCare HomeBase software is strongly preferred
  • Ability to maintain a structured, full-time schedule including daily patient visits
  • Reliable transportation and a valid drivers license
