Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with strong OASIS experience for a full-time, 13-week home health contract. This role is ideal for a nurse ready to deliver exceptional care, ensure accurate documentation, and manage a consistent schedule. Schedule:
- Full-time: Monday through Friday
- On-call coverage may be required
- Daily structured visit requirements
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to patients
- Perform thorough patient assessments and promptly complete OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to plan and manage individualized care
- Provide wound care, medication management, disease monitoring, and patient/family education
- Communicate promptly with the clinical team about patient status and any changes in condition
Preferred Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience in home health as an RN
- Proficiency with OASIS documentation is required
- Familiarity with HomeCare HomeBase strongly preferred
- Ability to maintain a structured, full-time schedule with consistent patient visits
- Reliable transportation and a valid drivers license
