Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with strong OASIS experience for a full-time, 13-week home health contract. This role is ideal for a nurse ready to deliver exceptional care, ensure accurate documentation, and manage a consistent schedule. Schedule:

  • Full-time: Monday through Friday
  • On-call coverage may be required
  • Daily structured visit requirements

Key Responsibilities:

  • Deliver in-home nursing care and skilled interventions to patients
  • Perform thorough patient assessments and promptly complete OASIS documentation
  • Collaborate with physicians and interdisciplinary care teams to plan and manage individualized care
  • Provide wound care, medication management, disease monitoring, and patient/family education
  • Communicate promptly with the clinical team about patient status and any changes in condition

Preferred Qualifications:

  • Active RN license in good standing
  • At least 1 year of recent experience in home health as an RN
  • Proficiency with OASIS documentation is required
  • Familiarity with HomeCare HomeBase strongly preferred
  • Ability to maintain a structured, full-time schedule with consistent patient visits
  • Reliable transportation and a valid drivers license
