We are seeking a dedicated Registered Nurse (RN) with strong home health experience and expertise in OASIS documentation for a full-time, 13-week contract assignment. This role is a chance to provide one-on-one care in patients homes while working alongside a collaborative and professional team.
Schedule:
- Full-time, Monday through Friday
- On-call rotation likely
- Structured daily visit expectations
Key Responsibilities:
- Deliver comprehensive in-home nursing care and interventions
- Conduct patient assessments and ensure timely, accurate OASIS documentation
- Work collaboratively with physicians and allied health professionals to tailor individualized care plans
- Perform wound care, medication administration, patient/caregiver education, and disease monitoring
- Proactively communicate any changes in patient condition to the clinical team
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a home health RN
- OASIS documentation proficiency is required
- Familiarity with HomeCare HomeBase documentation system (strongly preferred)
- Must be able to manage a structured, full-time schedule handling daily patient visits
- Reliable transportation and a valid drivers license essential
