Key Qualifications:
- Active RN license
- At least 1 year of recent experience in home health nursing
- Proven expertise with OASIS documentation
- Strong proficiency with HomeCare HomeBase (preferred)
- Reliable transportation and a valid drivers license
- Ability to independently manage a set caseload and meet scheduled visits
Primary Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Perform thorough patient assessments and accurate completion of OASIS documentation in a timely manner
- Work collaboratively with physicians and interdisciplinary teams to customize care plans
- Provide wound care, medication administration, patient education, and chronic disease monitoring
- Communicate changes in patient condition promptly to clinical team members, ensuring continuity of care
