Schedule & Contract Details:
- 13-week contract with potential for extension
- Full-time hours, Monday through Friday
- On-call responsibilities may be included
- Structured daily visit expectations
Core Responsibilities:
- Deliver in-home nursing care and perform skilled interventions for home health patients
- Conduct thorough patient assessments and complete OASIS documentation with precision and timeliness
- Collaborate with physicians and interdisciplinary teams to implement effective care plans
- Provide wound care, manage medications, offer patient education, and monitor chronic conditions
- Update clinical teams regularly with any changes in patient status
Qualifications:
- Active Registered Nurse (RN) license
- At least 1 year of recent experience as a home health RN
- Demonstrated proficiency in OASIS documentation
- Strongly prefer experience using HomeCare HomeBase software
- Ability to manage a full-time, structured schedule with daily travel
- Reliable transportation and a valid drivers license are required
