Schedule/Hours:
- Contract positionschedule and assignment details provided upon interview
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their residences
- Conduct both initial and ongoing assessments, monitoring vital signs, wound care, and medication management
- Develop, implement, and adjust individualized care plans based on patient needs
- Educate patients and their families on disease management, medication regimens, and preventive health practices
- Coordinate and communicate with physicians, therapists, and other healthcare professionals to ensure seamless care delivery
- Maintain accurate, timely documentation in electronic medical records (EMR)
- Closely monitor changes in patient condition and report relevant updates to supervising physicians
- Adhere to all applicable state and federal home health regulations
Qualifications:
- Active Registered Nurse (RN) license
- Minimum 3 months of recent experience in home health, including OASIS documentation
- Prior experience with Homecare Homebase (HCHB) is highly desirable
- Proficient clinical skills in IV therapy, medication management, and wound care
- Comfort with EMR systems
- Exceptional interpersonal and communication skills
- Independent time management and patient prioritization abilities
- Valid drivers license and reliable transportation
