We are seeking a dedicated Registered Nurse with strong home health experience, particularly skilled in OASIS documentation, for a 13-week full-time contract assignment. If you have a passion for patient-centered care, are meticulous with documentation, and excel at self-management, this position allows you to make a meaningful impact within patients homes.
Schedule:
- 13-week contract with possibility of extension
- Monday through Friday, full-time hours
- On-call responsibilities may be required
Key Qualifications:
- Current active RN license
- Minimum of one year recent RN experience in home health
- Proficiency with OASIS documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Ability to manage a consistent schedule and daily home visits
Primary Responsibilities:
- Deliver skilled nursing care and interventions in home settings
- Perform patient assessments and complete timely, accurate OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to implement patient-specific care plans
- Provide wound care, medication administration, disease monitoring, and patient/caregiver education
- Communicate status updates and changes in patient conditions effectively with the clinical team
