Schedule:
- 13-week contract with the potential for extension
- Full-time: Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Complete thorough patient assessments and OASIS documentation promptly and accurately
- Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans
- Perform wound care, medication administration, patient education, and chronic disease monitoring
- Maintain clear, timely communication regarding changes in patient conditions with the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent home health experience as a Registered Nurse
- Strong OASIS assessment and documentation skills
- Proficiency with HomeCare HomeBase (preferred)
- Ability to organize and handle a structured schedule with required daily visits
- Reliable transportation and valid drivers license
