Schedule/Hours:
- Contract position with flexible scheduling opportunities depending on patient needs
Qualifications:
- Active RN license
- Minimum of three months of home health experience utilizing OASIS
- Proficiency with electronic medical records (EMR) systems; HCHB experience considered a significant asset
- Strong clinical skills, including IV therapy, wound care, and medication management
- Excellent communication and interpersonal abilities
- Demonstrated ability to work independently and efficiently manage your caseload
- Valid drivers license and reliable transportation
Key Responsibilities:
- Conduct comprehensive nursing assessments and provide ongoing care in patients homes
- Develop and implement customized care plans based on thorough patient evaluations
- Educate patients and family members about disease management, medications, and preventive strategies
- Collaborate with physicians, therapists, and the wider healthcare team to ensure coordinated and effective care
- Maintain timely, accurate documentation of patient care activities within EMR systems
- Monitor patient condition and promptly report changes to supervising physicians
- Uphold compliance with all state and federal home health care regulations
