Key Responsibilities:
- Deliver thorough nursing care, including initial and ongoing patient assessments, vital signs monitoring, medication management, and wound care.
- Develop and execute individualized care plans tailored to patient needs.
- Provide education to patients and families on disease management, treatment plans, and prevention.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to ensure coordinated care.
- Accurately document all patient care activities using electronic medical records (EMR).
- Monitor patient conditions and promptly report changes to supervising physicians.
- Maintain compliance with all state and federal home health care regulations.
Qualifications:
- Active Registered Nurse license.
- Minimum of three months home health experience with proficiency in OASIS.
- Experience with HomeCare HomeBase (HCHB) is a strong advantage.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficient in EMR systems.
- Excellent communication and interpersonal skills.
- Able to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
