Job Description

A full-time Home Health Registered Nurse (RN) opportunity is available in Fairbanks, Alaska. This contract position offers the chance to provide comprehensive care to patients in a home health setting, focusing on individualized assessment and collaborative care planning.

Responsibilities include assessing patients physical, psychological, social, spiritual, educational, developmental, and cultural needs. You will review patient history and physical examinations, as well as diagnostic and laboratory data, reporting any abnormal results to physicians and relevant caregivers. Completing thorough assessments and reassessments according to patient needs and policy guidelines is essential. You will also formulate and initiate patient-specific plans of care that incorporate input from patients, families, and the healthcare team, ensuring goals are measurable and achievable. Care planning will involve collaboration with a multidisciplinary team to address the full spectrum of patient needs, including discharge planning.

Qualifications and Desired Experience:

  • Licensed Registered Nurse (RN) with at least 1 year of professional nursing experience
  • Strong assessment and critical thinking skills
  • Ability to work independently and coordinate with a multidisciplinary team
  • Cultural sensitivity and the ability to tailor care plans to diverse patient needs
  • Effective communication with patients, families, and healthcare providers

Schedule and Requirements:

  • Full-time, 8-hour shifts
  • Weekend on-call and holiday coverage on a rotation basis as needed
  • No floating requirements
