A full-time Registered Nurse (RN) is needed to provide home health care services in Ester, AK. This contract position involves delivering comprehensive patient assessments and coordinating care plans tailored to individual patient needs. The role focuses on evaluating physical, psychological, social, and cultural factors to develop realistic and measurable goals in collaboration with patients, families, and healthcare teams.
Responsibilities:
- Conduct thorough assessments covering physical, psychological, social, spiritual, educational, developmental, and cultural aspects.
- Review patient history, diagnostic tests, and laboratory results; communicate abnormalities to physicians and caregivers.
- Develop and initiate personalized plans of care and discharge plans based on assessment data and input from multidisciplinary teams.
- Complete ongoing assessments and reassessments in accordance with patient needs and organizational policies.

Qualifications / Desired Experience:
