Job Description
An experienced Registered Nurse (RN) with a strong background in home health nursing is needed for a contract role in Elgin, IL. The ideal candidate is passionate about delivering quality patient care and has hands-on experience with OASIS assessments.
Key Responsibilities:
- Deliver comprehensive nursing care in patients homes, including vital signs monitoring, medication management, and wound care
- Conduct initial and ongoing patient assessments and develop individualized care plans
- Educate patients and families about disease management, treatment protocols, and preventive strategies
- Collaborate effectively with physicians, therapists, and other healthcare professionals to coordinate care
- Maintain accurate and timely documentation of all patient care activities using electronic medical records (EMR)
- Monitor and communicate changes in patient condition to the supervising physician
- Ensure compliance with all applicable state and federal regulations governing home health care
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of 3 months home health nursing experience with OASIS proficiency
- Experience with Home Care Home Base (HCHB) software is a significant advantage
- Strong clinical competencies including IV therapy, wound care, and medication administration
- Proficient in electronic medical records (EMR) systems
- Excellent communication and interpersonal skills
- Ability to manage time effectively and work independently
- Valid driver's license with reliable transportation
