This role focuses on delivering comprehensive nursing services in patients homes, requiring expertise in OASIS to assess and manage patient needs effectively.
Key Responsibilities:
- Deliver skilled nursing care including vital sign monitoring, medication administration, and wound management.
- Conduct thorough patient assessments to inform and develop personalized care plans.
- Educate patients and families on disease processes, treatment adherence, and prevention strategies.
- Collaborate closely with physicians, therapists, and other healthcare providers to ensure continuity of care.
- Maintain precise and up-to-date documentation in electronic medical records (EMR).
- Alert supervising physicians to any significant changes in patient condition.
- Adhere rigorously to all regulatory requirements governing home health nursing.
Qualifications:
- Active and unrestricted RN license.
- At least 3 months of home health nursing experience with proficiency in OASIS assessments.
- Experience with Homecare Homebase (HCHB) software is highly desirable.
- Strong clinical competencies, including IV therapy, wound care, and medication management.
- Skilled in using EMR systems.
- Excellent communication skills and the ability to work independently.
- Valid drivers license and dependable transportation.
