Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with strong OASIS experience for a 13-week full-time home health contract. This role is ideal for clinically skilled professionals with a passion for independent patient care, precise documentation, and effective schedule management.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily patient visit expectations
Key Responsibilities:
- Deliver in-home nursing care and implement skilled interventions
- Perform thorough patient assessments and accurately complete OASIS documentation in a timely manner
- Develop and update individualized care plans in collaboration with physicians and interdisciplinary care teams
- Provide wound care, medication management, patient/family education, and monitor chronic disease status
- Communicate promptly about any patient condition changes to the clinical team
Qualifications:
- Active RN license (required)
- At least 1 year of recent experience as a home health RN
- Proficient with OASIS documentation (required)
- HomeCare HomeBase proficiency strongly preferred
- Able to independently manage a structured full-time schedule and daily visits
- Reliable transportation and a valid drivers license
