Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse (RN) with expertise in home health and OASIS documentation for a 13-week, full-time contract assignment. This role is perfect for skilled clinicians who excel in documentation accuracy, clinical judgment, and managing a structured visitation schedule.
Schedule:
- 13-week contract duration with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit quotas
Responsibilities:
- Deliver in-home nursing care, including skilled nursing interventions
- Conduct patient assessments and complete accurate, timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to create and follow personalized care plans
- Provide wound care, administer medications, and educate patients and families on disease management
- Monitor patient progress, identifying and communicating changes in condition to the clinical team
Qualifications:
- Current, active RN license
- At least one year of recent home health RN experience
- Proficiency in OASIS documentation
- HomeCare HomeBase experience is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to independently manage a full-time, structured visit schedule
