A Registered Nurse with home health expertise is needed to provide compassionate, patient-focused care in a contract role based in Dunn, NC. This position is ideal for an RN with hands-on experience in home health nursing and proficiency in OASIS documentation.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes
- Conduct initial and ongoing assessments including vital sign monitoring, medication management, and wound care
- Develop and implement personalized care plans tailored to each patients needs
- Educate patients and families on disease management, treatment plans, and preventive measures
- Collaborate with physicians, therapists, and other healthcare providers for coordinated care
- Accurately document all nursing care activities in electronic medical records (EMR)
- Monitor patient conditions and report changes promptly to supervising physicians
- Ensure compliance with home health regulations at state and federal levels
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of 3 months of home health experience working with OASIS
- Experience with Homecare Homebase (HCHB) is highly desirable
- Strong clinical skills including IV therapy, wound care, and medication administration
- Proficient with EMR systems
- Excellent communication and interpersonal skills
- Ability to work independently with effective time management
- Valid drivers license and reliable transportation
