A dedicated Registered Nurse (RN) with home health experience is needed for a 13-week full-time contract opportunity in Dublin, OH. This role focuses on delivering skilled nursing care in patients homes, utilizing clinical expertise and accurate documentation to support individualized care.
Responsibilities:
- Provide in-home nursing care and skilled interventions
- Conduct patient assessments and complete OASIS documentation accurately and on time
- Collaborate with physicians and multidisciplinary care teams to develop and implement personalized care plans
- Perform wound care, medication management, patient education, and disease monitoring
- Communicate updates and changes in patient condition to the clinical team promptly
Qualifications:
- Active RN license
- Minimum of 1 year recent home health RN experience with OASIS documentation expertise
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to manage a structured full-time schedule, including daily patient visits
- Reliable transportation and a valid drivers license required
Schedule:
- Full-time, Monday through Friday
- On-call duties probable
