Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with a background in Home Health and expertise in OASIS documentation for a full-time, 13-week contract. This position is ideal for professionals who thrive in autonomous environments, maintain exceptional clinical standards, and value structured, patient-centered care.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Complete thorough patient assessments and precise OASIS documentation
- Collaborate with physicians and multidisciplinary teams to implement individualized care plans
- Perform wound care, medication management, education, and ongoing disease monitoring
- Ensure accurate communication of patient status and care updates to the clinical team
Qualifications:
- Current, active RN license
- Minimum of one year of recent experience as a home health RN
- Demonstrated OASIS documentation proficiency
- Strongly prefer candidates with HomeCare HomeBase software experience
- Reliable transportation and a valid drivers license required
- Ability to maintain a structured schedule with daily visit requirements
