A dedicated Registered Nurse (RN) with home health and OASIS experience is needed for a full-time 13-week contract in Dequincy, LA. This role offers a meaningful opportunity to deliver skilled nursing care in patients homes while working within a structured schedule, Monday through Friday. Your clinical expertise and documentation accuracy will directly impact patient outcomes and provide essential support to a collaborative care team.

Responsibilities:
- Provide comprehensive in-home nursing care including skilled interventions and wound management
- Perform thorough patient assessments and ensure timely, accurate completion of OASIS documentation
- Collaborate with physicians and multi-disciplinary teams to implement individualized care plans
- Manage medication administration, patient education, and ongoing disease monitoring
- Communicate patient status updates effectively to the clinical team
Qualifications:
- Current RN license with at least 1 year of recent home health nursing experience
- Required experience with OASIS documentation and patient assessments
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to maintain a full-time schedule with daily patient visits
- Reliable transportation and a valid driver's license
