A rewarding opportunity is available for a Registered Nurse (RN) specializing in home health to provide compassionate, skilled care in a patients home. This 13-week full-time contract role requires expertise in OASIS documentation and clinical nursing assessments, offering a chance to impact patient outcomes directly while working within a structured schedule.

Schedule/Hours:
- Full-time, Monday through Friday
- On-call availability as needed
- Structured daily patient visits
Key Responsibilities:
- Deliver skilled nursing interventions and in-home care
- Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
- Collaborate closely with physicians and multidisciplinary care teams to implement individualized care plans
- Conduct wound care, medication management, patient education, and disease monitoring
- Communicate effectively regarding patient status and changes to the clinical team
Qualifications and Experience:
- Active RN license is required
- Minimum of one year recent experience in home health nursing
- Demonstrated proficiency with OASIS documentation
- Familiarity with HomeCare HomeBase software is strongly preferred
- Ability to manage a structured schedule with daily visits
- Reliable transportation and a valid drivers license
