Schedule/Hours:

  • Flexible hours with on-call responsibilities, ensuring a meaningful work-life balance.

Responsibilities:

  • Conduct skilled nursing assessments for patients in their homes, addressing physical, psychosocial, and environmental needs.
  • Develop, implement, document, and evaluate individualized Plans of Care, ensuring quality and continuity across services.
  • Obtain and incorporate data on physical, cultural, spiritual, and psychological factors that affect patient and family health.
  • Maintain detailed, up-to-date patient records, documenting all visits, care plans, interventions, and goals.
  • Utilize effective infection control measures per OSHA guidelines to protect yourself and your patients.
  • Educate patients and families on care plans, medications, diet, home safety, and promote self-care practices.
  • Supervise and provide clinical guidance to HHAs, LVNs, and STNAs.
  • Skillfully manage pain, utilizing evidence-based techniques and providing necessary education.
  • Coordinate referrals and ensure compliance with third-party payer requirements.
  • Communicate effectively with patients, families, healthcare team, and external agencies.
  • Participate in interdisciplinary team meetings and provide on-call services as assigned.

Qualifications:

  • Current state RN license and CPR certification (required); CHPN certification (preferred).
  • Graduate of an accredited nursing program (ADN required, BSN preferred).
  • Homecare Homebase and OASIS experience (required).
  • 2+ years home health experience and 1-2 years RN clinical experience (required).
  • Valid drivers license, reliable transportation, and insurance.
  • Strong clinical skills in wound care, IV therapy, venipuncture, equipment, and patient education.
