Responsibilities:
- Deliver in-home nursing care and skilled interventions aligned with personalized care plans
- Conduct thorough patient assessments and complete OASIS documentation accurately and on schedule
- Collaborate closely with physicians and healthcare teams to tailor treatments
- Perform wound care, medication management, patient education, and chronic disease monitoring
- Communicate patient updates effectively to maintain continuity of care
Qualifications and Desired Experience:
- Active RN license with a minimum of 1 year recent home health experience
- Required experience with OASIS documentation
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to maintain a structured daily schedule with multiple patient visits
- Reliable transportation and valid drivers license
Schedule:
- Full-time, Monday through Friday
- On-call duties probable
