An experienced Registered Nurse (RN) with a strong background in home health is needed for a contract position based in Decatur, AL. This role focuses on delivering high-quality, personalized nursing care in patients' homes, requiring OASIS experience to ensure comprehensive and compliant documentation and assessments.
Key Responsibilities:
- Provide thorough nursing care to patients in their homes, including initial and ongoing assessments.
- Manage vital signs, medication administration, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment protocols, and preventive care.
- Collaborate closely with physicians, therapists, and other healthcare professionals to maintain cohesive care.
- Accurately document all patient interactions in electronic medical records (EMR).
- Monitor patient conditions continuously and report any changes to the supervising physician.
- Adhere strictly to state and federal regulations governing home health care.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months experience in home health using OASIS.
- Experience with HCHB is highly desirable.
- Strong clinical proficiency in IV therapy, wound care, and medication management.
- Skilled in electronic medical records (EMR) systems.
- Excellent communication and interpersonal abilities.
- Capacity to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
