An opportunity is available for an experienced Registered Nurse (RN) specializing in home health care, offering a contract position in Darlington, SC. This role emphasizes providing high-quality, patient-centered nursing care in a home setting, with a focus on assessment, individualized care planning, and collaboration with multidisciplinary teams.
Key Responsibilities:
- Conduct comprehensive initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement customized care plans that address patient-specific needs.
- Educate patients and their families on disease management, treatment protocols, and preventive healthcare practices.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to maintain cohesive patient care.
- Accurately document patient care activities in electronic medical records (EMR).
- Monitor patient conditions and promptly report any significant changes to supervising physicians.
- Adhere to all applicable state and federal regulations governing home health care.
Qualifications:
- Valid and active Registered Nurse (RN) license.
- A minimum of three months of home health experience utilizing OASIS.
- Experience with the Homecare Homebase (HCHB) system is highly advantageous.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficiency in electronic medical records (EMR) systems.
- Excellent communication and interpersonal abilities.
- Capacity to work independently while managing time efficiently.
- Valid drivers license and reliable transportation are required.
