Job Description
We are seeking a skilled and compassionate Registered Nurse (RN) with home health experience to provide essential nursing care to patients in their homes on a contract basis. This role requires proficiency in OASIS assessments and a commitment to delivering high-quality, individualized patient care.
Location: Dallas, TX
Responsibilities:
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement personalized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Collaborate closely with physicians, therapists, and other healthcare professionals to ensure coordinated care.
- Accurately document all patient care activities in electronic medical records (EMR).
- Monitor patient conditions and report any changes to supervising physicians.
- Ensure compliance with state and federal home health regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months of home health nursing experience with OASIS documentation.
- Experience with HCHB is highly desirable.
- Strong clinical capabilities in IV therapy, wound care, and medication management.
- Proficient in EMR usage.
- Excellent communication and interpersonal skills.
- Ability to work independently while managing time effectively.
- Valid drivers license and reliable transportation.
