We are seeking a motivated and compassionate Registered Nurse (RN) with strong home health and OASIS documentation experience for a 13-week full-time contract assignment. Schedule:

• Full-time, Monday through Friday
• Probable on-call duties as needed
• Consistent daily visit requirements

Essential Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Accurate and timely OASIS documentation skills
• Proficiency with HomeCare HomeBase preferred
• Reliable transportation and valid drivers license
• Ability to manage structured schedules and daily patient visits

Primary Responsibilities:

• Deliver skilled nursing care and clinical interventions in patients homes
• Conduct initial and ongoing patient assessments, documenting through OASIS
• Develop and implement individualized care plans in collaboration with healthcare teams
• Provide wound care, medication management, patient education, and monitor chronic illnesses
• Communicate patient status and care updates promptly to the interdisciplinary team
