Schedule:
- Full-time, Monday through Friday
- Regular daytime hours with probable on-call rotation
- Structured daily patient visit expectations
Qualifications:
- Active RN license (in good standing)
- Minimum of 1 year recent experience as a home health RN
- Proficient and comfortable completing OASIS documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Dependable transportation and a valid driver's license
- Excellent communication and organizational skills
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to patients
- Perform timely and accurate patient assessments, including thorough OASIS documentation
- Collaborate effectively with physicians and interdisciplinary care teams to create and execute individualized care plans
- Provide wound care, manage medications, educate patients and families, and monitor disease processes
- Communicate changes in patient condition promptly to the clinical team
