A full-time, 13-week contract opportunity is available for a Registered Nurse (RN) with home health experience and proficiency in OASIS documentation. This role requires clinical expertise, strong documentation skills, and the ability to manage a structured schedule while making a positive impact on patient outcomes.
Location: Connersville, IN

Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Conduct thorough patient assessments and complete OASIS documentation accurately and in a timely manner
- Collaborate with physicians and interdisciplinary care teams to implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate patient condition updates and changes promptly to the clinical team
Qualifications:
- Current, active RN license
- Minimum of 1 year recent experience in home health nursing
- Experience with OASIS documentation is required
- Proficiency with HomeCare HomeBase software is strongly preferred
- Ability to manage a full-time schedule with daily patient visits
- Reliable transportation and valid drivers license
