Job Description

A full-time, 13-week contract opportunity is available for a Registered Nurse (RN) with home health experience and proficiency in OASIS documentation. This role requires clinical expertise, strong documentation skills, and the ability to manage a structured schedule while making a positive impact on patient outcomes.

Responsibilities:

  • Deliver skilled nursing care and interventions in patients homes
  • Conduct thorough patient assessments and complete OASIS documentation accurately and in a timely manner
  • Collaborate with physicians and interdisciplinary care teams to implement individualized care plans
  • Perform wound care, medication management, patient education, and monitor disease progression
  • Communicate patient condition updates and changes promptly to the clinical team

Qualifications:

  • Current, active RN license
  • Minimum of 1 year recent experience in home health nursing
  • Experience with OASIS documentation is required
  • Proficiency with HomeCare HomeBase software is strongly preferred
  • Ability to manage a full-time schedule with daily patient visits
  • Reliable transportation and valid drivers license
