An experienced Registered Nurse (RN) specializing in home health care is needed for a contract role in Concord, NC. This position requires a strong commitment to delivering compassionate and skilled nursing care in patients homes, with a focus on comprehensive assessments, personalized care planning, and patient education. Candidates must have OASIS experience to qualify.
Key Responsibilities:
- Provide thorough nursing care, including vital signs monitoring, medication management, and wound care in home settings.
- Conduct initial and follow-up patient assessments to evaluate health status.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and their families on disease management, treatment adherence, and preventive health measures.
- Collaborate with physicians and other healthcare providers to coordinate patient care.
- Accurately document all care activities in the electronic medical records (EMR) system in a timely manner.
- Monitor and report any changes in patient conditions.
- Ensure all home health care practices comply with state and federal regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- At least 3 months of home health nursing experience utilizing OASIS.
- Experience with Homecare Homebase (HCHB) software is highly desirable.
- Clinical skills including IV therapy, wound care, and medication administration.
- Proficient in EMR technology.
- Strong communication and interpersonal abilities.
- Ability to work independently and manage schedules effectively.
- Valid drivers license and dependable transportation.
