Job Description

An experienced Registered Nurse (RN) with a background in home health is needed for a contract position in Columbia, MO. This role requires a passion for patient care and proficiency in OASIS assessments to ensure high-quality, personalized care for patients in their homes.

Key Responsibilities:

  • Deliver comprehensive nursing care in patients home environments.
  • Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
  • Develop and follow individualized care plans tailored to patient needs.
  • Educate patients and families on disease management, treatment, and prevention.
  • Collaborate with physicians and other healthcare professionals for coordinated care.
  • Accurately document patient care activities in electronic medical records (EMR).
  • Monitor patient conditions and promptly report changes to supervising physicians.
  • Ensure adherence to all state and federal home health care regulations.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 3 months experience in home health utilizing OASIS.
  • Experience with Home Care Home Base (HCHB) software is highly desirable.
  • Strong clinical skills in IV therapy, wound care, and medication administration.
  • Proficiency using EMR systems.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Valid drivers license and reliable transportation.
