An experienced Registered Nurse (RN) with a background in home health is needed for a contract position in Columbia, MO. This role requires a passion for patient care and proficiency in OASIS assessments to ensure high-quality, personalized care for patients in their homes.
Key Responsibilities:
- Deliver comprehensive nursing care in patients home environments.
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and follow individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment, and prevention.
- Collaborate with physicians and other healthcare professionals for coordinated care.
- Accurately document patient care activities in electronic medical records (EMR).
- Monitor patient conditions and promptly report changes to supervising physicians.
- Ensure adherence to all state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months experience in home health utilizing OASIS.
- Experience with Home Care Home Base (HCHB) software is highly desirable.
- Strong clinical skills in IV therapy, wound care, and medication administration.
- Proficiency using EMR systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
