An experienced Registered Nurse with home health expertise is needed for a rewarding contract position in the Clarksville, IN area. This role focuses on delivering high-quality nursing care directly in patients homes, supporting their health and recovery through personalized care plans and ongoing assessments. Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes.
- Conduct initial and ongoing patient assessments including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment options, and preventive care.
- Collaborate with physicians, therapists, and other healthcare professionals for cohesive care delivery.
- Accurately document all patient care activities in electronic medical records (EMR).
- Monitor patient condition changes and report promptly to supervising physicians.
- Ensure compliance with state and federal regulations governing home health care.
Qualifications:
- Active Registered Nurse (RN) license.
- At least 3 months of home health experience with OASIS documentation.
- Experience with Homecare Homebase (HCHB) is highly desirable.
- Strong clinical skills including IV therapy, wound care, and medication management.
- Proficiency in electronic medical records (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver's license and reliable transportation.
