A rewarding opportunity is available for an experienced Registered Nurse (RN) specializing in home health care. This 13-week, full-time contract role offers the chance to deliver skilled nursing care in patients homes while utilizing your expertise in OASIS documentation. Location:

Schedule/Hours:
- Full-time, Monday through Friday
- On-call duties probable
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled interventions
- Accurately perform patient assessments and complete OASIS documentation timely
- Collaborate with physicians and healthcare teams to implement individualized care plans
- Manage wound care, medication administration, patient education, and disease monitoring
- Communicate patient condition updates to the clinical team
Qualifications & Desired Experience:
- Active Registered Nurse (RN) license
- Minimum of 1 year recent home health nursing experience
- Required proficiency in OASIS documentation
- Strongly preferred experience with HomeCare HomeBase software
- Ability to maintain a structured schedule with daily patient visits
- Reliable transportation and valid drivers license
