Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with strong home health and OASIS documentation experience for a 13-week full-time contract role. This position is ideal for a clinician who excels in independent home visits, accurate clinical assessments, and thorough documentation in a collaborative care environment.
Schedule:
- 13-week assignment with potential for extension
- Full-time, Monday through Friday
- Structured daily patient visits
- Possible on-call responsibilities
Key Responsibilities:
- Deliver skilled in-home nursing care tailored to individual patient needs
- Complete comprehensive patient assessments and accurate, timely OASIS documentation
- Collaborate closely with physicians and interdisciplinary team members to develop and implement personalized care plans
- Provide wound care, medication management, patient and family education, and disease process monitoring
- Communicate changes in patient conditions promptly to the clinical team
Qualifications:
- Valid, active RN license
- At least one year of recent home health RN experience required
- Proficiency in OASIS documentation is essential
- Experience with HomeCare HomeBase software highly preferred
- Strong organizational skills and ability to manage a full, structured patient visit schedule
- Reliable transportation and a valid drivers license
