Responsibilities:
- Deliver comprehensive nursing care as part of the home health and hospice team
- Conduct detailed patient assessments and coordinate care plans tailored to each individuals needs
- Utilize strong clinical judgment to manage pain, symptoms, and emotional care
- Collaborate effectively with interdisciplinary healthcare professionals
- Offer education and support to families and caregivers
- Accurately maintain patient records, with HCHB software proficiency considered an asset
- Provide ongoing communication and empathy throughout care transitions
Qualifications:
- Valid Registered Nurse license
- Minimum of 2 years experience in hospice or palliative care
- HCHB (HomeCare HomeBase) experience preferred
- Exceptional interpersonal, assessment, and clinical skills
- Demonstrated compassion and the ability to support patients and families during sensitive times
- Must possess reliable transportation and a current drivers license
