This opportunity is for a dedicated Registered Nurse (RN) with strong OASIS experience to join a full-time home health contract in Candler, North Carolina. Over a 13-week contract, you will provide skilled nursing care, complete thorough documentation, and collaborate with healthcare teams to deliver personalized patient care. Responsibilities:
- Deliver in-home nursing care and skilled interventions tailored to individual patient needs
- Conduct comprehensive patient assessments and complete OASIS documentation accurately and on schedule
- Collaborate with physicians and multidisciplinary care teams to implement and update care plans
- Administer wound care, manage medications, provide patient education, and monitor chronic conditions
- Communicate patient status updates and any changes to the clinical team promptly
Qualifications and Desired Experience:
- Active Registered Nurse (RN) license
- Minimum one year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation is required
- Experience with HomeCare HomeBase is strongly preferred
- Ability to manage a structured, full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
