A Registered Nurse with home health experience is sought for a 13-week full-time contract opportunity based in Cambridge, MD. This role requires a dedicated RN skilled in clinical practice and documentation, particularly with OASIS assessments. The position offers a structured schedule Monday through Friday with on-call duties likely, providing an excellent chance to deliver impactful patient care in a home health setting.
Key Responsibilities:
- Provide in-home nursing care and skilled interventions
- Conduct patient assessments and complete OASIS documentation accurately and timely
- Collaborate with physicians and care teams to create individualized care plans
- Perform wound care, medication management, patient education, and disease monitoring
- Communicate patient condition updates to the clinical team
Qualifications and Desired Experience:
- Active RN license
- Minimum of one year recent experience in home health nursing
- Required proficiency in OASIS documentation
- Familiarity with HomeCare HomeBase is strongly preferred
- Ability to manage a full-time schedule with daily patient visits
- Valid drivers license and reliable transportation
