Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse (RN) with strong OASIS skills for a full-time, 13-week home health assignment. If you thrive in independent environments, excel at clinical documentation, and appreciate the flexibility and impact of community-based care, this role is for you.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- Possible on-call responsibilities
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing services to patients in accordance with individualized care plans
- Complete thorough patient assessments, ensuring OASIS documentation is timely and accurate
- Collaborate closely with physicians, therapists, and care coordinators to adjust and implement patient care
- Perform clinical tasks including wound care, medication management, and disease-specific education
- Provide clear communication on changes in patient condition to the broader clinical team
Qualifications:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation (required)
- Proficiency in HomeCare HomeBase is strongly preferred
- Organized, with the ability to efficiently manage a busy schedule and maintain daily visit commitments
- Valid drivers license and reliable personal transportation
