An immediate contract opportunity is available for an experienced Registered Nurse with a background in home health care. This role focuses on delivering compassionate, high-quality nursing care to patients in their homes, ensuring patient safety, well-being, and effective care coordination. OASIS experience is required to succeed in this position.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care
- Develop and implement individualized patient care plans based on each clients needs
- Educate patients and family members on disease management, treatment protocols, and preventive healthcare
- Collaborate with physicians, therapists, and other healthcare professionals for cohesive care planning
- Accurately and promptly document patient care in electronic medical records (EMR)
- Monitor for, and report, changes in patient conditions to supervising providers
- Comply with all applicable state and federal regulations for home health care
Schedule/Hours:
- Contract-based position; hours determined by patient needs and assignment
Qualifications:
- Active Registered Nurse license (RN)
- Minimum of 3 months of recent home health experience utilizing OASIS documentation
- HCHB (Homecare Homebase) experience highly preferred
- Proficient clinical skills, including IV therapy, wound care, and medication administration
- Experience with electronic medical records required
- Excellent communication and organizational abilities
- Ability to work independently and manage a flexible caseload
- Valid drivers license and dependable transportation
