Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with solid experience in home health and OASIS documentation for a 13-week, full-time contract role. This position is ideal for RNs who excel at managing a structured schedule, providing high-quality care, and maintaining thorough documentation.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes, adhering to individualized treatment plans
- Complete patient assessments and accurate OASIS documentation in a timely manner
- Implement interventions including wound care, medication management, disease monitoring, and patient education
- Collaborate effectively with physicians and interdisciplinary care teams to ensure optimal outcomes
- Communicate patient status and care updates to the clinical team promptly
- Travel between patient locations and maintain professional reliability
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of one year recent RN experience in home health settings
- Strong proficiency with OASIS is required
- Prior experience with HomeCare HomeBase is highly preferred
- Reliable transportation and a valid drivers license
- Ability to independently manage a structured schedule and daily patient visits
