Qualifications & Experience:

• Active, unrestricted Registered Nurse (RN) license
• Minimum of one year direct clinical experience in Home Health or similar setting
• Proficient with electronic medical records and documentation standards
• Demonstrated ability to work independently and collaborate with multidisciplinary teams
• Strong interpersonal and communication skills, especially with diverse patient populations
• CPR/BLS certification

Key Responsibilities:

• Conduct comprehensive patient assessments and ongoing evaluations in the home setting
• Develop individualized care plans, in collaboration with patients, families, and other caregivers
• Administer prescribed medications, wound care, and other nursing interventions
• Educate patients and family members regarding diagnosis, treatment, self-care, and prevention
• Coordinate with physical therapists, social workers, and physicians as part of the care team
• Monitor patient progress and update care plans as needed
• Accurately document all clinical activities and maintain up-to-date patient records
• Adhere to infection control, safety guidelines, and regulatory requirements
