A skilled Registered Nurse with OASIS expertise is needed for a 13-week full-time home health contract. Schedule:
- 13-week contract, with extension potential
- Full-time, Monday through Friday
- Structured daily visits; some on-call responsibilities may apply
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Complete thorough patient assessments and timely OASIS documentation
- Work closely with physicians and interdisciplinary teams to coordinate individualized care plans
- Perform wound care, medication management, and disease monitoring
- Educate patients and their families regarding treatments and healthcare practices
- Communicate any changes in patient status promptly to the clinical team
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent experience as a home health RN
- Proficient with OASIS documentation (required)
- HomeCare HomeBase documentation software experience strongly preferred
- Able to manage a consistent full-time visit schedule
- Reliable transportation and a valid drivers license
