An experienced Registered Nurse (RN) with a passion for home health care is needed to provide high-quality nursing services on a contract basis. This role focuses on delivering personalized care to patients in their homes, emphasizing comprehensive assessments and collaborative care planning. Candidates with strong clinical expertise in wound care, medication management, and IV therapy are highly encouraged to apply, especially those proficient with OASIS documentation.
Key Responsibilities:
- Deliver thorough nursing care to patients within their home environments.
- Conduct initial and ongoing patient assessments, including vital signs and medication oversight.
- Create and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment strategies, and preventive health.
- Collaborate closely with physicians, therapists, and healthcare team members to coordinate patient care.
- Accurately document all care activities promptly in electronic medical records (EMR).
- Monitor patient conditions and report changes to supervising physicians.
- Maintain compliance with state and federal home health regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- At least 3 months of home health experience with OASIS documentation.
- Experience with Homecare Homebase (HCHB) software is an advantage.
- Strong clinical skills in IV therapy, wound care, and medication management.
- Proficiency with EMR systems.
- Exceptional communication and interpersonal abilities.
- Self-motivated with strong time management skills.
- Valid driver's license and reliable transportation.
