An opportunity is available for an experienced Registered Nurse (RN) specializing in home health to provide high-quality, patient-centered care on a contract basis. This role requires expertise in home health nursing and experience with OASIS assessments, focusing on delivering comprehensive care tailored to each patient's individual needs.
Key Responsibilities:
- Deliver thorough nursing care to patients in their homes, addressing a variety of health needs.
- Conduct initial and ongoing patient assessments including vital signs, medication management, and wound care.
- Develop and implement personalized care plans based on patient conditions and goals.
- Educate patients and their families on disease management, treatment protocols, and preventive health practices.
- Collaborate effectively with physicians, therapists, and healthcare team members to maintain cohesive care coordination.
- Accurately document all patient care activities in electronic medical records (EMR).
- Monitor patient status and report any changes to supervising physicians promptly.
- Ensure adherence to state and federal regulations governing home health care.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months of home health nursing experience with proficiency in OASIS assessments.
- Experience with Home Care Home Base (HCHB) software is highly desirable.
- Strong clinical competencies including IV therapy, wound care, and medication management.
- Skilled in electronic medical record systems.
- Excellent communication and interpersonal abilities.
- Self-motivated with effective time management skills.
- Valid drivers license and reliable transportation required.
