An opportunity is available for an experienced Registered Nurse (RN) specializing in home health to provide essential nursing care in patients residences on a contract basis. This role focuses on delivering personalized, high-quality care by applying clinical expertise and coordinating with a multidisciplinary healthcare team.
Key Responsibilities:
- Conduct comprehensive initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Create and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment protocols, and preventive strategies.
- Collaborate closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
- Accurately document patient care activities using electronic medical records (EMR) systems.
- Monitor patient status and report significant changes to the supervising physician.
- Adhere to all state and federal regulations governing home health care services.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months of home health experience with OASIS proficiency.
- Experience with Homecare Homebase (HCHB) software is a strong advantage.
- Strong clinical skills, especially in IV therapy, wound care, and medication management.
- Proficient in electronic medical records documentation.
- Excellent communication and organizational skills, with the ability to work independently.
- Valid drivers license and dependable transportation for patient visits.
