Job Description

A Registered Nurse (RN) with home health expertise is needed for a contract position based in Bessemer, AL. This role requires an experienced nurse skilled in providing in-home patient care, with a particular emphasis on OASIS assessments.

Key Responsibilities:

  • Deliver comprehensive nursing care to patients in their homes.
  • Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
  • Develop and implement personalized care plans aligned with patient needs.
  • Educate patients and their families on disease management, treatment plans, and preventive care.
  • Collaborate with physicians, therapists, and other healthcare professionals to maintain a coordinated care plan.

Qualifications:

  • Active RN license.

  • Proficient clinical skills, including IV therapy, wound care, and medication management.
  • Competent in using electronic medical records (EMR) systems.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Valid drivers license and reliable transportation.
