A Registered Nurse (RN) with home health expertise is needed for a contract position based in Bessemer, AL. This role requires an experienced nurse skilled in providing in-home patient care, with a particular emphasis on OASIS assessments.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes.
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement personalized care plans aligned with patient needs.
- Educate patients and their families on disease management, treatment plans, and preventive care.
- Collaborate with physicians, therapists, and other healthcare professionals to maintain a coordinated care plan.
- Accurately document patient care activities in electronic medical records (EMR).
- Monitor and communicate any changes in patient condition to the supervising physician.
- Ensure compliance with state and federal home health care regulations.
Qualifications:
- Active RN license.
- Minimum of 3 months home health experience using OASIS.
- Experience with Home Care Home Base (HCHB) software is advantageous.
- Proficient clinical skills, including IV therapy, wound care, and medication management.
- Competent in using electronic medical records (EMR) systems.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
