Job Description

Registered Nurses with home health experience are needed for contract positions based in Berwick, PA. This role requires a strong commitment to patient-centered care and expertise in home health nursing practices, including OASIS documentation.

Key Responsibilities:

  • Deliver comprehensive nursing care to patients in their homes.
  • Conduct initial and ongoing assessments, including monitoring vital signs, managing medications, and providing wound care.
  • Develop and implement personalized care plans tailored to patient needs.
  • Educate patients and families on disease management, treatment plans, and preventive health measures.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure integrated care.

  • Monitor patient conditions and promptly report changes to supervising physicians.
  • Ensure compliance with state and federal home health regulations.

Qualifications:

  • Active Registered Nurse (RN) license.

  • Experience with Homecare Homebase (HCHB) software is highly desirable.
  • Proficient clinical skills including IV therapy, wound care, and medication management.
  • Competent in electronic medical records (EMR) systems.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Valid drivers license and reliable transportation.
