Your next nursing opportunity awaits as a Registered Nurse with expertise in home health and a strong background in OASIS documentation. This full-time contract assignment offers the chance to work independently in a dynamic environment while enjoying the support of a dedicated care team.
Schedule:
- 13-week contract, potential for extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured schedule with daily home visits
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to diverse patient populations
- Complete thorough patient assessments and OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans
- Manage wound care, perform medication administration, educate patients and families, and monitor disease progression
- Maintain clear communication with both patients and the clinical team regarding any changes in condition
Required Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proficient in OASIS assessment tools and documentation
- Familiarity with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to manage a full-time, structured weekly schedule
