An opportunity is available for a Registered Nurse (RN) specializing in home health to provide patient-centered care in a contract role. This position requires a dedicated professional experienced in delivering nursing services to patients in their home environments, ensuring comprehensive and compassionate care.
Key Responsibilities:
- Conduct initial and ongoing patient assessments, including taking vital signs, managing medications, and performing wound care.
- Develop and implement personalized care plans based on individual patient needs.
- Educate patients and their families on disease management, treatment protocols, and preventive healthcare.
- Collaborate with physicians, therapists, and interdisciplinary healthcare professionals to coordinate optimal care.
- Accurately document all patient care activities using electronic medical records (EMR).
- Monitor patient conditions and communicate significant changes to the supervising physician.
- Maintain compliance with applicable state and federal home health regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months experience in home health nursing with proficiency in OASIS documentation.
- Experience with Homecare Homebase (HCHB) software is highly desirable.
- Competent in clinical skills such as IV therapy, wound care, and medication administration.
- Strong knowledge of electronic medical record systems.
- Excellent communication and interpersonal abilities.
- Capable of working independently and managing time effectively.
- Valid drivers license and reliable transportation.
